Sliding
fee scale
Through a grant from the Indiana State
Department of Health, the clinic is able to offer a sliding fee
scale for individuals and their families who meet financial
guidelines, and who are uninsured, under-insured or who can’t
afford to pay for an office visit.
Vaccines for children
The clinic participates
in the Indiana “Vaccines for Children” program which provides
children’s vaccines free of charge for families who do not have
health insurance or whose health insurance does not pay for
immunizations.
Any child can receive
their vaccines at the clinic and our goal is to help our families
have all their immunizations up to date.
Parents should bring a
copy of the child’s previous immunization record when visiting the
clinic.
