Welcome to Hancock Health’s patient portal. From here you can log in to your account to easily access your information, view and request appointments, see your health history and test results, send secure messages to your Hancock Physician Network provider, and view your medications online.

Please note, you must be at least 18 to enroll, and your email must be on file prior to enrollment.

HealthConnect Family Accounts

We can create Family Accounts by enrolling a parent, guardian or spouse as a Portal User, then attaching the children/spouse/senior parents to that account, provided we have a portal proxy form on file.

PLEASE COMPLETE A SEPARATE PROXY FORM FOR EACH FAMILY MEMBER YOU WANT TO BE CONNECTED TO YOUR PORTAL. IF THE CHILD IS 12 YEARS OLD OR OLDER, THEY MUST SIGN THE FORM AS THE PATIENT.

Troubleshooting

If you have trouble logging in, first make sure that you have properly enrolled. If you have an account but have forgotten your password or ID, you can request new ones from the sign in page.  You can also review our Frequently Asked Questions section.
